Our consultant pharmacist, Ly can conduct Home Medicines Review for our patients who are on complex medicine regimens with multiple comorbidities. Ly currently works at Frankston Hospital as a clinical pharmacist and has over ten years of clinical experience. She also practices as a community pharmacist and has a wealth of experience and knowledge to provide expertise advice on the quality use of medicines. The HMR service is a comprehensive clinical review of a patient’s medicines aiming to achieve optimal medicine regimen as well improving the patient’s knowledge and understanding about medicines. The process is a collaboration between your GP and the consultant pharmacist working together to develop an appropriate medication management plan. The service is fully funded by Medicare (which mean no out of pocket cost to the patient) and best of all take place in the convenience of the patient’s home at a suitable time.
